  • High School Report Card/Form 138 (original copy)
  • Certificate of Good Moral Character (original copy)
  • NSO/PSA authenticated birth certificate (photocopy)
  • Form 137 (to be requested by UA Registrar from previous school)
  • Medical Certificate*
  • Admission Test Result

Transferees

  • Transcript of Record for evaluation (photocopy)
  • Honourable Dismissal/Transfer Credential (original copy)
  • Certificate of Good Moral Character (original copy)
  • NSO/PSA authenticated birth certificate (photocopy)
  • Official Transcript of Record copy for UA (to be requested by UA Registrar from former school)
  • Medical Certificate*
  • Admission Test Result

Shiftees

  • Approved Program Shifting Application Form
  • Admission Test result (as required by the program)
  • Medical Certificate*
